TBGHealth provides residential brain injury specialty services in Montgomery County and Centre County, PA. All services are provided in suburban areas in residential neighborhoods. TBGHealth operates private homes and apartments in Montgomery and Centre Counties.
Our programs offer 24-hour residential support, 7 days per week, for up to 3 individuals in a home setting and up to 2 individuals in a private apartment setting. Resident Care Aides are utilized to provide supervision and support in meeting individual goals as specified in the individual’s Rehabilitation Plan. If specified in an individual’s plan, TBGHealth will also provide 1:1 support.
Residential Habilitation Services include:
- Assisting individuals to acquire the basic skills necessary to maximize their independence in activities of daily living
- Assisting participants with acquiring, retaining and improving self-help, socialization, and adaptive skills
- Cueing and reminders
- Modeling of behavior
- Assistance in developing or maintaining maximum independent functioning in community living activities
- Assistance with activities of daily living (ADLs), including, but not limited to: toileting, bathing, dressing, eating, mobility, transfers, tooth brushing, medication administration
- Assistance with instrumental activities of daily living (IADLs), including, but not limited to: cooking, housekeeping, attending appointments, shopping, using the telephone, medication management
- Laundry services or assistance with completing laundry
- Three meals daily, plus beverages and snacks
- Housekeeping services or assistance with completing housekeeping activities
- Recreation and social activities, and transportation to such activities
- Transportation to Structured Day Program, medical appointments, religious services, agency-sponsored recreation activities
- Other transportation may be arranged, as needed by the individual
All services are provided directly to the resident by TBGHealth staff.
